Enrolment Application Form

The application is designed to assess your suitability in the Certificate in Applied Sports Nutrition program.

Every candidate has to complete this form, and no place in an intake is guaranteed.

We are looking to ensure that every student is able to have the most positive and successful learning experience, and to do so we are assessing your:

• Ability to study online in a self paced capacity with due dates
• Background with nutrition subject matter (to determine if additional texts are required)
• The manner in which you approach learning, and working with our assessment team

Instructions: Before you commence your course, we need some information to help us understand your literacy, language and numeracy skills and identify if you may require extra support during the training program.

Please read the instructions carefully before starting.

• This is not a test. Instead, it helps us to find the right type of course for you and identify where you may need additional support throughout your studies.
• A trainer or staff member will explain why we have this review.
• Please have a go at all the questions. Feel free to ask a trainer for help if you need it. Take your time and relax.
• After you have finished, a trainer or staff member will talk with you about:
  • If you need any extra help to do your course.
  • What you need to do next to start the course.

Please make sure you answer any contextual/scenario questions in the detail that would be expected from an ideal student in the program.
